Online Forms

This page provides library clients with links to forms for requesting Depository service. A brief description of each form follows below. If you have any questions about these forms, please call the HD Representative at (617) 496-3438. These order forms are not for use by University records storage clients. Records clients must contact Records Management Services of the Harvard University Archives (hul.harvard.edu/rmo/contact.html). Please also note that access to the actual Online Forms is restricted to computers on the Harvard network. Access to non-Harvard visitors is restricted solely to current HD clients who use the proper User Name and Password.

Media Pickup Request Form
To schedule transfers of new material or returns/refiles to the Depository.

Media Retrieval Request Form
To request retrievals of media from Depository storage.

Permanent Withdrawal of Media Form
To notify the Depository of items that are being withdrawn permanently from storage.

Barcode Label Order Form
To order new barcode labels.

Container Supply Order Form
To order storage boxes or media transfer containers.
